Enrolled individuals receive only those Services identified through individualized assessment as required to meet the individual's needs and team, approved by the Interdisciplinary Individuals enrolled in the HICS program may choose where and with whom to live, including the option of living with their natural family. Support services may be provided for only a few hours per week for individuals residing in their own home or with their natural family, or up to 24 hours per day for individuals who require total care and assistance.

Eligibility

  • To be eligible for HCS an individual must:

    • Have been determined by a licensed or Certified Psychologist or Physician to have mental retardation and be in need of institutional care unless supportive and habilitative services can be provided in his/her community.

    • Be eligible for Supplementary security income (SSI) and receive SSI benefits and continues to be eligible for Medicaid as mandated by federal law.

    • Individuals residing in state schools who desire to reside in the community.
